An Extension used to reference a specific athenahealth group-appointment Appointment.
A group appointment combines all Appointments that occupy the same “slot group” into a single representation of an appointment.
It is represented as a FHIR Appointment having an Appointment.id defined by a-{PID}.GroupAppointment-{ID} where {PID} is the Practice ID and {ID} is the Slot Group ID
